An exciting role has become available for a Recovery Coordinator within our Intensive Support Team. The Intensive Support Team sits within our Severe Multiple Disadvantage (SMD) Programme, offering high level support for those that have additional barriers in accessing services.

 

Where: Nottinghamshire

Hours: Full time, 37.5 hours per week

Contract Type: Fixed Term Contract until 31st March 2028

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

With this role you’ll be working as part of our Intensive Support Provision, covering the south of Nottinghamshire.

The team supports individuals who experience issues around drugs and/or alcohol, as well as developing wrap around support packages to ensure holistic approaches are taken to enable people to achieve the best possible outcomes.

About the role:

  • To manage a case load of service users who have been accepted via our Intensive Support Panel
  • Build relationships with and support individuals to access the support they
  • To work in close partnership with our core and housing team, as well as external partners
  • To manage service user plans and risk
  • To advise service users regarding their substance/alcohol use, using recognised theoretical models (e.g. Motivational Interviewing), supporting them to identify and explore concerns before deciding on a possible course of action.
  • To work with Mental Health Agencies to ensure that where appropriate, service users have access to address their mental health issues.
  • To work proactively to ensure that service users have access to ongoing
  • To convene and contribute to service user case conferences where
  • To work closely with all partner services, sharing appropriate information and ensuring that action plans are carried through.
  • To represent the team at external forums where appropriate and requested to do so.
  • To maintain accurate and comprehensive service user records in line with Change Grow Live's procedures and protocols and the requirements of the local authority, maintaining confidentiality and sharing information with appropriate agencies.
  • To participate in patterns of shift work to enable the team to maximise potential contact with service users, which will include working unsociable hours.

About you:

  • Knowledge of substance misuse, alcohol misuse, mental
  • Understanding of service users who have multiple and complex
  • Understanding the importance of multi-agency working within a strength based, asset building approach and maintaining professional boundaries
  • Experience of facilitating multi-agency meetings
  • Experience of and a commitment to, working positively in partnership with a range of statutory and voluntary agencies.
  • Good administrative, recording and reporting
  • Knowledge of safeguarding procedures applicable to children and vulnerable
  • Excellent communication, negotiation and advocacy
  • Confidence to build and maintain relationships with a wide range of services, representing the organisation and creating effective pathways
  • Confidence to work autonomously as well as part of the
  • The ability to work with confidence and patience with clients unwilling to engage with services and those that exhibit behaviours that challenge.
  • Good level of IT literacy, including the ability to use IT systems, including Microsoft Office applications
  • Ability to critically reflect and evaluate interventions, service delivery and areas for development
  • Full driving license and access to a road worthy vehicle
