We are looking for passionate and proactive Recovery Coordinators - Rough Sleeping to join a dedicated multi-disciplinary outreach team working across West Sussex (Drug & Alcohol Response Team DART).

This exciting role focuses on engaging people who are experiencing rough sleeping, or who are at risk of rough sleeping, and supporting them to access drug and alcohol treatment, harm reduction interventions and wider recovery support.

 

This is a rewarding and varied role for someone who is resilient, compassionate and motivated to improve outcomes for people facing multiple disadvantage. Working flexibly across community and street-based settings, you will build trusting relationships, reduce barriers to treatment and provide assertive outreach to individuals who we might usually struggle to engage with traditional services.

 

You will manage a caseload of service users, completing assessments, recovery planning, risk management and referrals, while working closely with homelessness services, housing providers, health services, criminal justice partners and other community organisations. You will play a key role in supporting continuity of care during accommodation moves, hospital admissions and discharges, or transitions from custody. 

 

The team works across the whole county. You will be based from one of our hubs in Worthing, Chichester, Bognor Regis or Crawley, covering a designated geographical area while supporting colleagues across the wider service when required.

You should be comfortable working outdoors in different weather conditions, travelling and adapting your approach to meet people where they are. 

 

No two days are the same. We're looking for someone who is curious, adaptable and a skilled communicator, with a commitment to person-centred, trauma-informed practice. Alongside direct work with service users, you'll maintain accurate records, contribute to safeguarding and risk management processes and support service development through partnership working and shared learning.

Responsibilities

About the Role

In this role, you will:

  • Engage people experiencing rough sleeping through planned outreach and joint working with homelessness services.
  • Build trusting, strengths-based relationships that support sustained engagement in treatment and recovery.
  • Deliver harm reduction and overdose prevention interventions, including naloxone provision and safer-use advice.
  • Complete assessments, recovery plans and risk assessments, supporting individuals to achieve their goals.
  • Facilitate rapid access to treatment, prescribing services, healthcare and wider support.
  • Work collaboratively with housing, health, criminal justice and community partners to coordinate care.
  • Support people through key transitions, including accommodation moves, hospital discharge and release from custody.
  • Maintain accurate records and contribute to safeguarding, information sharing and multi-agency risk management. 

What We Are Looking For

Essential

  • Experience of working with people affected by substance use and an understanding of evidence-based treatment, harm reduction and recovery approaches.
  • Experience of supporting people with multiple and complex needs, including homelessness, rough sleeping or social exclusion.
  • Excellent communication, engagement and relationship-building skills.
  • Ability to build rapport with individuals who may be reluctant to engage with services.
  • Confidence working independently, managing a caseload and prioritising competing demands.
  • Experience of partnership and multi-agency working.
  • Understanding of safeguarding, risk management and professional boundaries.
  • Good IT skills, including the ability to accurately maintain records and use Microsoft Office applications.
  • Ability to work flexibly across community settings, including occasional early mornings, evenings or weekends when required. 

Desirable

  • Experience of outreach or street-based working.
  • Knowledge of local homelessness, housing and support services.
  • Experience of supporting people through hospital discharge, prison release or accommodation transitions.
  • Knowledge of West Sussex local services and geography.
  • Driving licence and access to a vehicle.
