Our Team Leaders here at Change Grow Live play an integral role in ensuring an effective service provision for our service users by creating a focussed culture, engaging staff to provide an innovative and continually improving service and provide leadership, support, and development to a wide range of staff.   

 

 

We are currently seeking an enthusiastic and dynamic Team Leader - Early Intervention & Prevention to join our Kirklees team. As a Team Leader at Change Grow Live, you will be at the forefront of our service delivery. Your role is crucial in fostering a user-focused culture, motivating staff to provide innovative and continuously improving services, and offering leadership, support, and development to a diverse team.

 

Where: Kirklees

Hours: 37.5 hours per week

Contract: Permanent 

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. 

Responsibilities

Key Activities:

  • Encourage and lead the team with compassion, using attentive observation and practice to enhance the experiences and outcomes for service users. Provide feedback that is prompt, values-driven, and supportive.

  • Guide the team to promote hope, empowerment, choice, control, and opportunities that support service users in reaching their full potential as individuals and active community members.

  • Work closely with the management team to maintain strong governance and oversight, ensuring consistent excellence in service delivery and exceeding professional standards through best practice implementation.

  • Maintain accurate and detailed records of interactions with service users and professionals to ensure continuity and quality of care.

  • Lead and deliver on projects that increase visibility, create awareness, and improvement engagement including development of educational programmes, informative resources, and prevention pathways.
  • Be responsible for proactively generating and increasing referrals from relevant partners including utilising data to target specific services/provisions supported by your team.
  • Ensure effective and consistent service user involvement including coproduction.

  • Knowledge and experience of developing and leading health and social care services.
  • Knowledge and experience of delivering services which evidence high performance and quality standards.
  • Experience of culture and change management, staff engagement, service improvement.
  • Experience of effective partnership working, relationship management, pathway and protocol development.

To excel in this role:

  • Ability to lead, inspire and develop a team.
  • Expertise in developing positive working cultures and relationships, embedded in CGL values.
  • Skills which demonstrate effective communication in a variety of formats to a broad audience.
  • Confidence in working alone while following local risk management policies.
  • Strong problem-solving skills, especially in tough situations.
  • Development of programmes aimed at children and young people i.e. PHSE/RHSE work.
  • An understanding of the Stronger Families Programme and Early Support Partnership in Kirklees.
