We are looking for a motivated and compassionate Young Person’s Resilience Worker to join our Nottingham service, with a dedicated Youth Hubs function. This is an exciting opportunity to play a key role in shaping how young people access support across the city.

 

This role combines frontline intervention work with a visible, hub-based presence, supporting young people affected by substance use and other vulnerabilities, while strengthening pathways between services and professionals.

 

You will work directly with young people delivering evidence-based interventions, while also acting as a key link within the Youth Hubs —improving accessibility, supporting referrals, and helping professionals navigate pathways into the service.

 

Where: Nottingham City

Contract: Fixed Term until 30th of September 2028.

Hours: Full time, 37.5h per week

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

Requirements

  • Experience working with young people in a supportive or therapeutic capacity
  • Knowledge of substance misuse, safeguarding, and harm reduction
  • Ability to work flexibly in community settings and multi-agency environments
  • Strong communication and partnership-working skills
  • Understanding of working within a hub or integrated service model (desirable)
  • Hold a full current UK Driving License with access to own transport or be able to prove the ability to travel between services efficiently.

 

Responsibilities

About the role:

  • Support young people from entry into the service through their treatment and recovery journey
  • Deliver screening, assessments, psychosocial interventions, and care planning
  • Build trusted relationships and deliver person-centred, evidence-based interventions
  • Identify and respond appropriately to safeguarding concerns
  • Reduce drug- and alcohol-related harm for individuals and the wider community
  • Promote positive outcomes in health, wellbeing, and resilience

Youth Hubs-focused responsibilities:

  • Provide a consistent and visible presence within the Youth Hubs, making services accessible and welcoming for young people
  • Act as a Single Point of Contact for professionals, supporting clear communication and coordination across services
  • Support and streamline referral pathways, ensuring timely and effective access to support
  • Offer consultation, guidance, and informal training to professionals around substance use, risk, and engagement
  • Build strong partnerships with Nottingham City services to deliver a joined-up, multi-agency approach
  • Identify gaps, emerging needs, and opportunities to improve service delivery within the hub model

About you:

  • Passionate about supporting young people to achieve positive change
  • Strong understanding of safeguarding, including Fraser Guidelines and Gillick Competence
  • Experience working with young people affected by substance use or related vulnerabilities
  • Confident working across multiple agencies and building professional relationships
  • Strong communication, organisation, and record-keeping skills
  • Able to work proactively, independently, and as part of a team in a dynamic environment
