As an Assessment, Detox and Rehabilitaiton coordinator in with Change Grow Live you’ll work with people who use our services, helping them get to where they want to be.

 

Where: Liverpool 

Hours: Fulltime, 37.5 hours per week

Contract Type:  Permanent

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

With this role you’ll be working as part of River Liverpool Drug and Alcohol service , based in the city center but may be requires to work across the liverpool city area.

Why this role matters 

Your focus will be on assessing new servcie users into treatment and supporting them to access residential detox and/or Rehabilitation, providing effective case management and coordination of the individual recovery plans – with specific reference to detoxification / Rehabilitation preparation and support during the detoxification and rehabilitation period and linking in with our partner agencies who provide residential detox and/or Rehabilitation servcies. 

This is a new role for Liverpool focusing on developing the gateway for people who use our services to link into detox and Rehabilitation and is an exciting role to be part of developing new pathways and building partnerships.

 

Who we're looking for

You'll bring a combination of operational experience and knowledge to the role, supporting Service Users on the next stage of their journey.

You'll have:

  • Experience working within drug and alcohol services or a related health and social care environment
  • Demonstrable knowledge of clinical prescribing issues.
  • Knowledge and Experience of supporting delivery of community and residential detox programmes.
  • Experience of delivering evidence based psychosocial interventions on both a group and one-to-one basis.
