We are seeking a strategic and collaborative individual to support the integration of Change Grow Live services within Camden’s Integrated Neighbourhood Teams (INTs). In this pivotal role, you will scope the opportunities for the Camden Drug and Alcohol service to be fully embedded within neighbourhood-based multidisciplinary teams, strengthening how we support individuals with multiple disadvantages.

 

You will be scoping the opportunity to build strong partnerships across health, social care, primary care and voluntary sector organisations, to influence and share how the Camden service can add value to working within the integrated system. This will involve attendance at INT meetings in person and virtually. Your work will help to improve accessibility and quality of service delivery within the community. This role will require you to use initiative and take ownership of the development of the project.  

 

By embedding substance misuse expertise within neighbourhood teams, you will play a key role in reducing drug and alcohol related harm, improving outcomes and contributing to tackling health inequalities across Camden.

Where: Camden

Hours: Full Time. 37.5 per week

Contract: Fixed term for 12 months from start date 

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

 

Responsibilities

Key Responsibilities:

  • Seek opportunity to embed CGL services and staff within Integrated Neighbourhood Teams across Camden.
  • Build and maintain strong relationships with partners including Primary Care Networks, Adult Social Care, Mental Health services, Housing, Community Health Services and VCSE organisations.
  • Support the development of integrated referral pathways and improve access to treatment through neighbourhood working.
  • Promote the role and value of drug and alcohol services within multidisciplinary teams.
  • Enable joint working approaches such as MDT participation, shared care planning and joint visits.
  • Identify and resolve barriers to effective integration across services and systems.
  • Represent the Substance Misuse service at neighbourhood, partnership and operational meetings.
  • Work with internal teams to support workforce development and embed integrated ways of working.

 

Requirements:

  • Experience of working within substance misuse services or with people with multiple disadvantages.
  • Experience of partnership working across health, social care or community services.
  • Strong understanding of referral pathways and integrated working approaches.
  • Ability to build effective relationships with a range of stakeholders.
  • Experience of supporting or implementing service development or change.
  • Strong communication skills and the ability to work across multidisciplinary teams.
  • Ability to identify challenges and develop practical solutions within complex systems.
