About the Role
We are delighted to welcome applications for this innovative and exciting role, working in close partnership with Warwickshire County Council Children & Families Service. As Targeted Interventions Coordinator, you’ll join dynamic and passionate teams committed to prevention, early intervention, and targeted support for children and young people. This pivotal role focuses on coordinating substance misuse interventions for young people who are vulnerable or at risk, helping to create positive, lasting change at a crucial point in their lives.
Who we are:
Evolve is Change Grow Live’s confidential service in Warwickshire supporting young people between the ages of 5 and 25. We offer advice and guidance to young people who are affected by a loved one’s drug or alcohol misuse or if they are using drugs or alcohol themselves and want support to make changes.
The details
*Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Key Responsibilities
You will coordinate support for young people from referral through to exit, ensuring they access the right service at the right time. This includes screening and assessment, delivering brief and targeted interventions, and developing integrated care plans. You’ll work closely with partner agencies to ensure effective substance misuse pathways are in place, reduce drug‑ and alcohol‑related harm, and strengthen outcomes for young people, families, and communities. The role also involves advocacy to support access to wider services, promoting service user and carer involvement, delivering training and advice to Warwickshire Children’s Social Care teams, and responding appropriately to safeguarding concerns in line with statutory guidance.
