Your work will help people take those first steps into support — and crucially, stay engaged on their journey.

What we are looking for

We are looking for someone who:

• Has experience working with individuals affected by substance use, particularly opiate use
• Can deliver structured, evidence-based interventions
• Communicates confidently, both verbally and in writing, with good IT skills
• Is proactive, organised, and able to manage their own workload effectively
• Has a strong understanding of mental health and associated support pathways
• Works well as part of a team and builds strong relationships with a wide range of stakeholders
• Can adapt to change while keeping service users at the centre of everything they do
• Understands and adheres to information governance and safeguarding processes
• Is committed to ongoing learning and development

Please note: A full driving licence and access to a vehicle is essential for this role due to the community-based nature of the work.

About the role

In this role, you’ll be based within our Birmingham service while also working out in the community, building strong partnerships with local organisations, community groups, faith settings, and other agencies.

You will:

• Hold a caseload and deliver interventions to reduce harm related to substance use, including opiates, non-opiates, and alcohol
• Focus on re-engaging individuals who may have disengaged from treatment or are at risk of discharge
• Support service users to access appropriate prescribing pathways, including OST and alcohol detox, in line with CGL governance
• Work collaboratively with GPs, pharmacists, housing providers, and partner agencies to achieve the best outcomes
• Promote recovery, resilience, and independence, supporting individuals to rebuild and reconnect with their communities
• Offer flexible, community-based appointments, including working within temporary accommodation settings

Where: Birmingham
Hours: Part Time, 7.5 hours per week
Salary:* £27,861.26 – £32,002.35 (salary will be pro rata)**

*Salary is subject to review in line with the 2025/26 pay award.
Contract Type: Permanent

**Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
