Substance Misuse Link Worker

As a Substance Misuse Link Worker with Change Grow Live you’ll work with people who use our services, helping them get to where they want to be.

The Changing Futures Programme strives to improve the lives of those with multiple unmet needs (such as substance misuse, rough sleeping, mental health issues and offending behaviour) in Hull through effective and sustained engagement with the use of a multi- disciplinary and person-centred approach.

We are seeking a Substance Misuse Link Worker to work collaboratively within the Changing Futures Team, sharing and learning with colleagues from across partner organisations such as DWP, Criminal Justice, Mental Health and Adult Social Care.

Your work will be underpinned by the Making Every Adult Matter (MEAM) approach and to support and influence systemic change

Where: Hull

Full-Time Hours: 37.5 per week 

Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. 

About the role:

• To provide advice, information and motivational support to enable people to make informed decisions regarding their substance use
• To be skilled in using evidence-based techniques delivered on a 1:1 or group work basis to improve health, well-being and promote recovery.
• To provide personalised assessments and develop and regularly review holistic recovery and support plans with the person you and other professionals to reduce barriers to recovery and ensuring their needs are met.
• To keep and maintain records on both Changing Futures and ReNew’s case management systems.
• To be involved in the delivery of harm reduction approaches, e.g. Needle & Syringe Provision (NSP), Blood Borne Virus (BBV) testing and Take-Home Naloxone as required, supporting the Health Equity Agenda.
• To work as part of an integrated, multi-disciplinary team.
• Maintain a high standard of knowledge in the subject matter expert and continually update and share this knowledge.
• To be the single point of contact for all ReNew staff supporting people ‘under’ Changing Futures.

About you: 

• Experience of working in the health and social care sector with vulnerable people
• In-depth knowledge and experience of working with the complexities
• Understanding of people who have multiple unmet need
• Knowledge and experience of working in line with NICE guidelines Alcohol Pathways
• Understanding the importance of multi-agency working within a strength based, asset building approach and maintaining professional boundaries
• Experience of facilitating multi-agency meetings
• Experience of and a commitment to, working positively in partnership with a range of statutory and voluntary agencies.
• Good administrative, recording and reporting skills.
• Experience and knowledge of safeguarding procedures applicable to children and vulnerable adults.
• Experience of case management.
