As a Substance Misuse Link Worker with Change Grow Live you’ll work with people who use our services, helping them get to where they want to be.
The Changing Futures Programme strives to improve the lives of those with multiple unmet needs (such as substance misuse, rough sleeping, mental health issues and offending behaviour) in Hull through effective and sustained engagement with the use of a multi- disciplinary and person-centred approach.
We are seeking a Substance Misuse Link Worker to work collaboratively within the Changing Futures Team, sharing and learning with colleagues from across partner organisations such as DWP, Criminal Justice, Mental Health and Adult Social Care.
Your work will be underpinned by the Making Every Adult Matter (MEAM) approach and to support and influence systemic change

Full-Time Hours: 37.5 per week
Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
