We’re looking for a passionate and motivated Hospital Liaison Recovery Worker to join our Barnet Service. With this role, you’ll be working as part of the Change Grow Live Hospital Liaison team, based within local hospitals and A&E departments, acting as a vital link between acute services and community drug and alcohol services.

 

You’ll work closely with hospital staff, building strong relationships across wards and A&E, offering advice, guidance and training on substance misuse, and helping identify alternatives to unnecessary admissions. You’ll also play a key role in supporting people who present repeatedly, working with wider services to understand what’s going on and what might help break the cycle.

 

 

Location: Barnet

Hours: full time, 37.5 per week

Full time Salary Range: £27,861.26 - £32,002.35 dependent on experience, (based on full time working hours, pro rata for part time hours)

Allowance: Outer London Weighting (£2,195.72)

 

Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, salary and payments are pro rata based on contracted hours.

 

This role includes weekend and out-of-hours working, in line with hospital and service requirements.

Responsibilities

What You’ll Do

  • Build effective communication and referral pathways between the hospital and community addiction services.

  • Act as the key liaison for acute referrals, providing timely and professional advice to hospital teams.

  • Deliver alcohol and drug assessments using recognised screening tools (e.g. AUDIT), offering brief interventions and onward referral.

  • Develop strong working relationships across A&E, Gastroenterology, Hepatology, and other specialist wards.

  • Provide advanced assessment, planning and evaluation for a small, complex caseload — ensuring safe transitions from hospital to community care.

  • Support hospital staff in identifying and managing patients with substance use issues, offering guidance, awareness sessions and training.

  • Work with community recovery teams to ensure assertive and effective follow-up of repeat presentations and discharges.

  • Participate in service audits and research to improve practice and outcomes.

About You

  • Strong understanding of substance misuse and its impact on health and wellbeing.

  • Experience delivering brief interventions or working within hospital, health or substance-use settings.

  • Excellent communication and relationship-building skills — able to work confidently with hospital colleagues, patients and partner agencies.

  • Proactive and able to work autonomously while maintaining strong team connections.

  • Skilled in prioritising workload, managing competing demands and working compassionately in fast-paced environments.

  • Knowledge of safeguarding and mental health practice.

  • A commitment to continuous learning and improving outcomes for people we support.
