We're seeking a dedicated Hospital Liaison Worker to join our compassionate team in Liverpool- committed to supporting individuals at both the Royal Liverpool and Aintree Hospitals.

Where: Liverpool 

Part Time Hours: 37.5 hours per week

*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. 

Responsibilities

About the role: 

With this role you’ll be working as part of the hospital inreach team  based at both Liverpool and Aintree Hospitals.

The post holder will work across both hospital sites (e.g. A&E and admissions wards) to provide rapid access to assessment, brief interventions/harm minimisation and support discharge planning/onward referrals for people attending hospital with substance misuse related issues

  • Working closely with Recovery Co-ordinators and partner agencies to develop pathways to care 
  • Support service users in opiate and alcohol interventions within a hospital setting.
  • You will support on harm reduction interventions along with rapid access to treatment assessments.
  • Keeping the people who use our services at the heart of everything you do
  • Facilitate the training in the hospitals with the professionals 

 

About you: 

  • Good interpersonal skills and a willingness to work flexibly as part of a team
  • A clear understanding of the need for and ability to deliver quality services
  • Experience of working within substance misuse or a social care setting is essential
  • Have a strong understanding of substance misuse issues and experience of working within a similar field
  • Experience of working with groups/pods and managing group dynamics, such as conflict or challenging behaviour.
  • Be an excellent communicator, both verbal and written with good IT skills
  • Proactive and able to work on own initiative with excellent time management and prioritising skills
  • Have a good working knowledge of mental health interventions, services, and good practice
  • Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
  • Ability to manage change successfully in a way that prioritises the needs of service users
  • Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
  • Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
  • Good organisational skills
  • Confidence to work on own initiative as well as part of the team
  • Communicate confidently and effectively, verbally and in writing
  • Respond flexibly to the demands of the post
