Where: Dudley 

Hours: Full time, 37.5 per week

Contract Type: Fixed Term Contract until 31/3/2029 

 

*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. 

Responsibilities

About the Role

This is an exciting and pivotal post combining relationship‑focused liaison work with oversight of service data and performance. You will be the bridge between our service and primary care partners, ensuring communication flows smoothly and data is accurate, meaningful and used to improve outcomes.

GP & Pharmacy Liaison

  • Build and maintain positive, professional relationships with GP practices, PCNs and pharmacies.
  • Be the central contact for queries, referrals, treatment updates and collaborative working.
  • Promote awareness of our services, pathways and the support available for people we work with.
  • Work closely with prescribers and clinicians to help resolve issues and support safe, coordinated care.
  • Strengthen communication channels to make the journey easier for service users and partners.

Data Lead

  • Oversee data quality, reporting and performance monitoring across the service.
  • Champion good information governance, ensuring accurate use of Client ID where required.
  • Produce clear, insightful reports for managers, staff, commissioners and external partners.
  • Support and train staff to improve data entry accuracy, system use and compliance.
  • Identify trends, gaps and opportunities that support continuous improvement.

About You:

  • Knowledge and understanding of drug and alcohol issues and experience of working within a related field.
  • Knowledge and understanding of the role of pharmacies in the drug and alcohol treatment provision.
  • Knowledge and experience of multi-agency working and establishing
    partnerships with other professions to achieve good outcomes for the service user group, e.g. mental health services, criminal justice agencies, community groups, etc.
  • Experience of working in an outcomes-focused environment, and working with multiple demands.
  • Ability to communicate complex information to individuals and groups in a way that is easily understood.
  • Strong interpersonal skills, and the ability to build meaningful and productive relationships with other agencies, maintaining a highly cooperative approach to supporting colleagues in delivering service objectives.
  • To use data that contributes to service user safety and maximise outcomes.
  • Advanced knowledge of Microsoft Excel
  • Experience of maintaining Information Systems
  • Experience of managing large datasets and analysis
  • Highly IT Literate with good knowledge of Microsoft Office applications
  • Knowledge of the management of data in health and social care sectors, including substance misuse.
  • The ability to write reports and carry out analysis of information, presenting information to be understood by a range of stakeholders.
  • Excellent time management with the ability to prioritise and produce high quality outputs.
