We are pleased to offer an exciting opportunity for a Project Manager to join the River service in Liverpool, with a specialist remit for Entry into Treatment and Tier 4 Inpatient Pathways. Based at our Henry Street hub, you will play a central role in ensuring people can access treatment quickly, safely and with the right level of support throughout their journey.

 

This role is ideal for someone with strong operational leadership skills, excellent partnership capability, and a passion for improving treatment access and high‑intensity pathways for people with complex substance use needs.

Full Time Hours: 37.5 hours per week 

Full Time Salary: £39,891.91 - £44,471.72*  

Contract Type: Permanent 

 

*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. 

 

Responsibilities

About the Role

  • Lead the operational oversight of Entry into Treatment processes, ensuring they are accessible, trauma‑informed, timely and aligned with Liverpool’s treatment strategy.
  • Coordinate and oversee Tier 4 inpatient pathways, ensuring clear clinical governance, safe transitions and excellent communication with inpatient detox/rehab providers.
  • Provide leadership and reflective supervision to a multidisciplinary team delivering assessments, care coordination and high‑intensity support.
  • Strengthen and maintain strong partnership working with Mersey Care, acute hospitals, criminal justice partners, residential detox/rehab providers and community organisations.
  • Ensure that safeguarding practice remains robust and that risk is managed effectively across all treatment stages.
  • Use performance data and local intelligence to drive improvements in access, retention and outcomes.
  • Support staff in delivering high‑quality, evidence‑based interventions and care planning.
  • Ensure compliance with CGL policies, clinical standards and contractual requirements.
  • Promote a culture of hope, empowerment and person‑centred practice across the Entry into Treatment and Tier 4 functions.

About You

  • Experience supporting adults with substance misuse or multiple‑complexity needs.
  • Strong knowledge of treatment access processes, care coordination and Tier 4 pathways.
  • Proven leadership or project management experience in a health, social care or substance use environment.
  • Excellent understanding of harm reduction, recovery‑focused practice and trauma‑informed approaches.
  • Confident safeguarding knowledge and experience embedding safe operational practice.
  • The ability to build trusted relationships across partner agencies.
  • Strong organisational skills and the ability to work in a fast‑paced, responsive environment.
  • Excellent communication skills — written, verbal and relational.
  • A commitment to improving treatment access, equity and outcomes for people across Liverpool.
