We’re looking for a dedicated Adults Think Family Recovery Coordinator to join our Halton Integrated Drug and Alcohol Recovery Service. The post-holder will carry a caseload and act as the care coordinator for service users who have children or young people living in the household with them.
Interventions and support will be delivered via a whole family approach working with external support agencies and the YP service for hidden harm interventions. The team will take a systemic approach to improving the wellbeing of the family.
You will be working with individuals to develop positive lifestyle changes and health choices. You will be promoting harm reduction and personalised health promotion interventions to encourage people to access mainstream services within the community.

Full Time Hours: 37.5 per week
Full Time Salary: £27,861.26 - £32,002.35*
Contract Type: 12 Months Fixed Term Contract
*Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
About the role:
Provide assessment, psychosocial interventions, integrated care planning and after care support, including transfer of care.
Provide service user advocacy to support engagement from and access to partnership services.
To support the development of strong relations with partner agencies, ensuring robust referral protocols and care pathways for the effective identification,
referral and support for those requiring ‘hidden harm’ interventions from the
service.

25 days of annual leave, plus bank holidays. Additionally, you'll receive one extra day of annual leave for each year of service during your first five years, therefore you will enjoy 30 days of leave after five years with us.
