We are looking to recruit a Senior Practitioner to join our dedicated Criminal Justice Team in Nottinghamshire. You will ensure a high quality and consistent service to service users with complex needs who are involved in the criminal justice system. You will act as a practice lead within the team, supporting high standards of care, participating in audit cycles and championing best practice within the team.

 

Travel across Nottinghamshire will be required to meet the needs of service users.

 

Where: Nottinghamshire. Primary base negotiable: Hucknall, Mansfield or Worksop

Hours: Full Time, 37.5h  per week 

Full Time Salary: £32,002.35 - £34,214.20 per annum, paid pro rata

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role:

  • Identify, and engage with, people who have substance use problems with complex physical and / or mental health needs, including co-morbidity. Providing harm reduction interventions, advice, support and recovery focused structured psychosocial interventions to support recovery and primary health care needs.
  • Carry out comprehensive assessments and develop recovery plans that clearly identify how they will enable progression in addressing substance use and re/integrating into the community.
  • Ensure that all aspects of confidentiality are adhered to and ensure consent to share information is always obtained.
  • Work closely with criminal justice partners such as prisons, the police, the courts, probation and other external support agencies to ensure the effective coordination of care. 
  • Providing advocacy for access to partnership services
  • Ensuring quality of service delivery through data analysis and engaging staff in service quality improvement planning. Where necessary, create team and individual performance plans to improve quality and performance.
  • Support staff to improve service user outcomes through audits and observed practice, providing timely values-based feedback.
  • Deputise for the Team Leader/Project Manager in meetings, when necessary.
  • When applicable, manage the recruitment, supervision and development of Peer Mentors and Volunteers. 

About you:

  • Have a strong understanding of substance use issues and experience of working within the substance use field, ideally within criminal justice.
  • Be an excellent communicator, both verbal and written with good IT skills
  • Proactive and able to work on own initiative with excellent time management and prioritising skills.
  • Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders.
  • Ability to manage change successfully in a way that prioritises the needs of service users.
  • Understand the importance of information governance processes and commit to follow and apply all necessary safeguards.
  • Ability to participate in case audit cycles.
  • Can provide training and guidance to colleagues, sharing knowledge around harm reduction and relevant interventions.
