PHE Homeless Recovery Coordinator

Change, Grow, Live services and staff provide wraparound support to rough sleepers and will work directly with emergency accommodation to support the staff with this group of people. We are looking for enthusiastic individuals and friendly faces to help support people in leading healthier lives.  

A fantastic, new opportunity has arisen to become a PHE Homeless Recovery Coordinator, we are now looking to recruit a fixed term staff member to join our dedicated multi-disciplinary team in Birmingham to support rough sleepers with harm reduction and to engagement in treatment and will sit within drug and alcohol treatment services. 

You will focus on engaging with people who have a history of rough sleeping and substance misuse. You will have some experience of engaging with individuals with multiple and/or complex needs and demonstrate a determined approach to finding solutions.  

Where: Birmingham

Hours: Full time, 37.5 per week

Salary Range: £27,861.26 - £32,002.35 dependent on experience (based on full time hours, pro rata for part time hours) 

Contract Type: Fixed Term Contract until 31/12/2026

*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. 

About the role:

• Establishing effective communication and referral pathways those individuals who are homeless and in contact with other health and care agencies
• Work closely with the local authority, Health & Social care sector, partners and any provision linked to Homelessness and improve joint working, attending multi-disciplinary meetings when as and when required
• Supporting service users from point of entry into the service and through their treatment/recovery journey
• Providing screening, assessment, and recovery planning and onward referral
• Coordinate the transfer of any accommodated out of area individual. This may include referral to and liaison with their new drug and alcohol service and pharmacy and provision of bridging prescription until an appointment and prescription is provided. Promoting carer, service user and community involvement
• Providing advocacy for access to partnership services
• Working with service users to support social (re)integration, enabling them to lead meaningful and purposeful lives: promoting recovery, resilience, peer support and self-determination

About you:

• You will focus on engaging with people who have a history of rough sleeping and substance misuse. You will have some experience of engaging with individuals with multiple and/or complex needs and demonstrate a determined approach to finding solutions. Some knowledge of one or more of the following is required:   
  • Homelessness  
  • Mental health  
  • Criminal Justice System  
  • Substance misuse  
• Have a strong knowledge and understanding of substance misuse issues
• Experience of engaging with a wide variety of individuals with a variety of backgrounds and needs
• Be an excellent communicator, both verbal and written with good IT skills
• Proactive and able to work on own initiative with excellent time management and prioritising skills
• Have a good working knowledge of mental health interventions, services, and good practice
• Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
• Ability to manage change successfully in a way that prioritises the needs of service users
• Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
