Are you passionate about helping people rebuild their lives? We're looking for a dedicated Outreach / Hub Complex Case Worker to support individuals facing homelessness and rough sleeping. In this role, you'll provide intensive, person-centred support, helping people overcome barriers and access vital services. You'll work both on the streets and in accommodation settings, offering flexible interventions and empowering individuals to make sustainable, positive changes. Hours: 37.5 hours per week, Full Time
Full time Salary: £27,861.26 - £32,002.35 (based on full time hours, pro rata for part time)
Contract Type: 12 Months Fixed Term
Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours
About the Role:
About You:
Experience in the health and social care sector supporting individuals with complex needs, including substance use and mental health.
Strong knowledge of at least one area: substance use, alcohol use, mental health, housing, or offending.
Understanding of severe and multiple disadvantage and effective support approaches.
Awareness of multi-agency working and the importance of maintaining professional boundaries within a strengths-based framework.
Experience facilitating and contributing to multi-agency meetings and partnerships across statutory and voluntary sectors.
Competent administrative, recording, and reporting skills.
Knowledge and experience of safeguarding for children and vulnerable adults.
Understanding of supported housing and experience in case management.
