Are you passionate about helping people rebuild their lives? We're looking for a dedicated Outreach / Hub Complex Case Worker to support individuals facing homelessness and rough sleeping. In this role, you'll provide intensive, person-centred support, helping people overcome barriers and access vital services. You'll work both on the streets and in accommodation settings, offering flexible interventions and empowering individuals to make sustainable, positive changes. Where: Leeds

Hours: 37.5 hours per week, Full Time

Full time Salary: £27,861.26 - £32,002.35 (based on full time hours, pro rata for part time)

Contract Type: 12 Months Fixed Term 

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours

Responsibilities

About the Role:

  • To co-ordinate and lead a pro-active solution focussed approach to support people identified that have or are currently experiencing rough sleeping, with a view to engage them in appropriate services and move them away from a street-based lifestyle.
  • To identify and establish contact with people experiencing rough sleeping and a street-based lifestyle, working within protocols to rapidly assist them through street outreach shifts at a variety of times and a range of locations.
  • To carry a case load of individuals where their needs are not being met and develop personalised action plans (identifying goals and aspirations) that ensure these individuals are less likely to rough sleep in future.
  • To review everyone's action plan and facilitate them to lead in the process.
  • To assist people that are experiencing homelessness to be discharged from establishments such as hospital and prison at an appropriate time by referring them into appropriate accommodation as quickly as possible.
  • To work proactively to ensure that the people you support have access to ongoing healthcare.
  • To divert people who are new to experiencing rough sleeping when appropriate and to encourage and assist people to return to their home communities.
  • To meet challenging targets involving accessing sustainable accommodation options for people and providing regular reports on progress.
  • To participate in street counts when requested.
  • To participate in patterns of shift work to enable the team to maximise potential contact with service users, which will include working unsociable hours, weekends, and in severe weather conditions.

About You:

  • Experience in the health and social care sector supporting individuals with complex needs, including substance use and mental health.

  • Strong knowledge of at least one area: substance use, alcohol use, mental health, housing, or offending.

  • Understanding of severe and multiple disadvantage and effective support approaches.

  • Awareness of multi-agency working and the importance of maintaining professional boundaries within a strengths-based framework.

  • Experience facilitating and contributing to multi-agency meetings and partnerships across statutory and voluntary sectors.

  • Competent administrative, recording, and reporting skills.

  • Knowledge and experience of safeguarding for children and vulnerable adults.

  • Understanding of supported housing and experience in case management.
