Change Grow Live in Waltham Forest are looking for a Senior Recovery Coordiantor - Mental Health Practitioner to support the development of the Shared care services and improve our relationships with pharmacies. You will work to identify and support those individuals who present with co-existing mental health and substance misuse problems i.e. dual diagnosis service users.

 

This role will be pivotal in strengthening our ambitions around dual diagnosis, and ensuring mental health services for people with a Serious Mental illness truly embody a “no wrong door” approach. The role will play an important part in designing what the future of what joint working and collaboration between mental health services and CGL will look like in the future.

 

Where: Waltham Forest

Full Time Hours: 37.5 hours per week

Full Time Salary Range: £32,002.35 - £34,214.21 Dependent on experience, pro rata for part time hours

Allowance: Outer London Weighting £2,195.72 (pro rata)

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role: 

  • Provide bespoke interventions for identified service users, with the aim of engaging people in the most appropriate service for their needs, reducing re-presentations and bringing about positive client outcomes.
  • Support the development of joint working protocols with Primary Care, mental health services and substance misuse services (including: Community Mental Health Teams (CMHTs), Improving Access to Psychological Therapies services).
  • Identify the training and support needs of colleagues working with this group of service users and deliver training or provide support as appropriate.
  • Assess suitability for referrals into services in terms of risk and specific needs.
  • To work closely with all partner services, sharing appropriate information and ensuring that action plans are carried through.
  • To maintain accurate and comprehensive service user records in line with Change Grow Live's procedures and protocols and the requirements of the local authority, maintaining confidentiality and sharing information with appropriate agencies.

About you: 

  • Excellent knowledge and understanding of drug and alcohol issues and experience of working within a related field. 
  • Knowledge and experience of multi-agency working and establishing partnerships with other professions to achieve good outcomes for the service user group. 
  • Ability to communicate complex information to individuals and groups in a way that is easily understood. 
  • Written and verbal communication skills and a high degree of personal IT competency. The ability to accurately update and maintain records in a timely fashion and to work to deadlines for the submission of information. 
  • Excellent team working and interpersonal skills, maintaining a highly cooperative approach to supporting colleagues in delivering service objectives.
