As a Connecting Communities Case Worker with Change Grow Live you’ll work with people in custody, who use our services, to prepare for transition back into the community.This is an excellent opportunity for someone with experience of substance misuse issues, looking to take the next step in their career with a charity that will support your continued personal and professional development.  

  

This role is based across 3 prison sites, HMP Elmley, HMP Swaleside & HMP Standford Hill.

 

Full Time Salary Range: £27,449,52 to £31,529,41 (pro rata for part time hours)

Full Time Hours: 37.5 per week

Contract: Permanent

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

 

This is a prison based role and is subject to prison clearance.

Responsibilities

Change Grow Live currently provide psychosocial interventions and transitional services across prisons in Kent. This contract includes the Connecting Communities team. This team prepares people engaging with CGL and Oxleas for release back into the community or when transferring to another site.    

    

Dedicated staff will work together with healthcare colleagues supporting clients when transitioning to communities across the UK. Our enhanced offer will include flexibility to engage with clients for a period of time post release.  

   

You will join a dedicated and motivated team who are passionate about rehabilitation and recovery. We are looking to recruit a passionate and committed individual to the role of Case Worker within the Connecting Communities Team based within the following three prisons:   

   

HMP Elmley is a Remand adult male prison  

   

HMP Swaleside is a category B long term prison   

   

HMP Standford Hill is a category D open prison for males.
