We're seeking a dedicated Reach Worker to join our Kent team. The post holder will deliver high quality service to groups who traditionally find our services challenging to access and use the Making Every Contact Count (MECC) approach.

 

This will involve providing assertive outreach and in-reach work to enable service users and potential service users to access treatment services.

 

Where: West Kent - Gravesend, Tunbridge Wells, Maidstone.

Full Time Hours: 37.5 per week

Full Time Salary: £27,861.26 - £32,002.35 per year dependant on experience 

Contract Type: 9 Months Fixed Term with possibility of extension. 

 

*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. 

Responsibilities

The Role:

As a valued member of our team in Kent, you'll be taking on a central role with responsibilities including:
  • To deliver substance misuse outreach services to key places and identified groups and promote harm reduction and swift access for those who want to engage with treatment.
  • Work in close partnership with Accident & Emergency (A&E) Depts and other hospital teams to develop discharge pathways and plans for hospital frequent attenders
  • Develop specific links with identified specialist acute services and provide in-reach to hospital wards.
  • Focus on early intervention and assertive engagement for people who are identified as needing additional support.
  • Work closely with a wide range of partner agencies including GPs, Social Workers, Community Mental Health Teams, regularly attend Community Safety/Vulnerability Forums and other multi agency meetings
  • Ensure case files demonstrate high quality interventions, MECC, SMART goals and clear outcome informationProvide drop-in clinics in community locations
  • Conduct dynamic risk assessments and compile robust health and safety and risk management plans

About You:

To thrive in this role, we're looking for someone who has:

  • Experience of working within a multidisciplinary team and outreach locations
  • Ability to work on a lone worker basis and stay calm under pressure
  • Proven experience of client assessment, case management, and care co-ordination
  • Experience of conducting dynamic risk assessments and the ability to compile robust health and safety and risk management plans
  • Have emotional resilience and the ability to employ diplomatic and pragmatic approaches to managing priorities and difficult situations
