Overview

As a part of Change Grow Live team you’ll help people get to where they want to be and make a difference in their lives.  

 

Change, Grow, Live services and staff will provide wraparound support to rough sleepers and will work directly with emergency accommodation to support the staff with this group of people. We are looking for enthusiastic, dynamic individuals and friendly faceto help support people in leading healthier lives.  

 

A fantastic opportunity has arisen to become an Assertive Drug & Alcohol Outreach Workerengaging with rough sleepers with the aim of moving them into more settled lifestyles. 

 

The post holder will work as part of the wider Hull ReNew service to work towards preventing homelessness and supporting effective treatment pathways, work with people in a variety of settings to ensure engagement with drug and alcohol treatment services.

 

Where: Hull

Hours: Full time, 37.5 per week

Full Time Salary Range: £27,861.26 - £32,002.35 dependent on experience (pro rata for part time hours)

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

You will focus on engaging with people who have a history of rough sleeping and substance misuse. You will have some experience of engaging with individuals with multiple and/or complex needs and demonstrate a determined approach to finding solutions. Some knowledge of one or more of the following is required:  

  

Homelessness  

Mental health  

Criminal Justice System  

Substance misuse  

 

About the Role:

  • A flexible and creative approach to contacting and working with all people who have dropped out of treatment services.
  • Attend relevant multi-disciplinary team meetings supporting links to health and social care services, peer support networks, community assets and mutual aid.
  • Work closely with the local authority, Health & Social care sector, partners and any provision linked to Homelessness and improve joint working
  • Liaise with other services involved in the care and support of an individual to ensure appropriate and secure transfer of all care needs.
  • Coordinate the transfer of any accommodated out of area individual. This may include referral to and liaison with their new drug and alcohol service and pharmacy and provision of bridging prescription until an appointment and prescription is provided.
  • Take part in regular outreach sessions to engage people who don’t feel ready to access the mainstream service.
  • Ensure there are culturally appropriate interventions for hard-to-reach individuals from the Eastern European and Sub-Saharan African rough sleeping communities.
  • Utilise existing/new networks and pathways to broker support from strategic partnerships and other providers.
  • Ensure there is continuity of care for those moving out of the area
  • Hold cases for a minimum of 4-6 weeks following a transition to ensure the service user moves between the two services successfully.

About You:

  • Experienced in outreach, community engagement, or working with vulnerable and excluded populations.

  • Skilled at building rapport and trust with individuals from a variety of backgrounds.

  • Confident in liaising and negotiating with partner agencies and professionals.

  • Knowledgeable about substance misuse, mental health, and social exclusion.

  • Able to work autonomously and manage your time effectively.

  • Resilient, adaptable, and solution-focused in challenging situations.

  • Passionate about equality, inclusion, and reducing health inequalities.
