We’re excited to offer a brand-new opportunity as an Targeted Intervention Coordinator to support targeted intervention and prevention work within a secure estate setting. Based at HMYOI Wetherby, The post holder will take the lead in delivering and coordinating impactful early interventions for individuals and groups identified as being at risk of harm from substance misuse.
You will actively champion the service’s targeted offer, ensuring it is widely recognised and embraced by partners, and will drive the development and maintenance of robust referral pathways that guarantee swift, straightforward access to treatment.
Where: HMYOI Wetherby with occassional travel to Leeds and Sheffield
Hours: 37.5 per week, Full Time
Full Time Salary Range: £27,861.26 - £32,002.35 dependant on experience (based on full time hours, pro rata for part time)
* please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
About the role:
Lead the coordination and delivery of the service’s Targeted Offer across the three secure estates.
Increase visibility and accessibility of the service among children, young people, families, and partners to drive referrals for specialist support.
Coordinate and deliver group work, training, and other relevant programmes.
Build and maintain strong working relationships with key partners to ensure high-quality delivery.
Support the creation of projects that boost visibility, raise awareness, and improve engagement, including educational programmes, informative resources, and prevention pathways.
Develop and implement innovative approaches to maximise engagement within the prison regime.
Design and deliver group work and training sessions to promote service awareness and early intervention messages.
Complete the Comprehensive Healthcare Assessment Tool (CHAT) with all new admissions.
About you:
We are looking for someone with:
Experience of working with young people in one-to-one and/or group settings.
Strong knowledge of issues affecting young people today, particularly around substance use.
Understanding of safeguarding concerns relating to children and young people.
Background in working within an outcomes-focused environment.
Proven ability to build effective working relationships with partners.
Experience of meeting key performance indicators (KPIs) relevant to the role.
Skilled in conducting needs assessments with children and young people.
Experience of delivering training to diverse audiences in a range of settings.
Full driving licence and access to a car.
Experience of managing projects and working to deadlines.
Willingness and ability to obtain full prison vetting clearance.
Desirable Criteria:
Experience supporting children and young people within secure estate settings.
Background in developing educational content tailored to young people.
Experience working with young people with additional needs, including SEND and SEMH, with the ability to adapt approaches accordingly.
A recognised Youth Work qualification or equivalent.
