This is an exciting opportunity to join Change Grow Live as an Assertive Outreach Worker, supporting people who are not currently accessing treatment services. You’ll play a key role in reaching and engaging some of the most vulnerable and hard-to-reach individuals in the community — including those experiencing homelessness, mental health challenges, or entrenched substance use.
Using assertive and creative outreach approaches, you will help service users move towards recovery and stability, while also developing strong partnerships with local agencies to create seamless pathways into treatment. This is a dynamic and rewarding role where no two days are the same. You’ll be working directly with some of the most vulnerable and hard-to-reach members of the community, including those who may be experiencing homelessness, mental health issues, or entrenched substance use.
You will also be facilitating training to outside agencies on the support we offer as a service. This role will also cover attending external events to promote our service, this may sometimes be on a weekend or evening.

Hours: Full time, 37.5h per week
Full Time Salary: £27,861.26 - £32,002.35 dependent on experience, pro rata fo part time hours
Contract Type: Fixed term until 31st March 2026
Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
Identify and proactively engage individuals not currently accessing treatment services, particularly those who are hard to reach or disengaged.
Develop and maintain strong, collaborative relationships with a wide range of partner agencies (e.g. housing providers, criminal justice services, mental health teams, and community organisations).
Deliver assertive outreach in community settings, including streets, hostels, day centres, and other non-traditional environments.
Conduct comprehensive assessments and support individuals into appropriate treatment and recovery pathways.
Advocate for service users to ensure they have access to the services and support they need.
Work flexibly and creatively to overcome barriers to engagement.
Maintain accurate and timely case records in line with service protocols.
Contribute to service development through feedback, collaboration, and innovation.

Able to work autonomously and manage your time effectively.
