Full Time Hours: 37.5 per week 

Full Time Salary Range: 37,184.90 - 44,471.72 (based on full time hours, pro rata for part time) 

 

*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. 

Responsibilities

The post-holder will be a specialist social worker, allocated to the Adult Social Care Hospital Discharge Team (Hull Royal Infirmary and Castle Hill Hospital), with a focus on enhancing current provision and increasing safe and timely hospital discharges. This role will primarily support patients with alcohol and/or substance misuse issues who require care and more intensive support upon discharge.

The social worker will:

  • Support vulnerable adults with alcohol and/or substance misuse needs in accessing appropriate community services (e.g. mental health, substance misuse, housing) following hospital discharge.
  • Facilitate safe and effective discharge planning, ensuring individuals are connected with the right support to meet their health and social care needs.
  • Work collaboratively with hospital teams and community partners to reduce the risk of readmission and promote long-term recovery and stability.
