We're seeking a dedicated Hospital In Reach Recovery Coordinator to provide a vital link between our Change Grow Live community service and hospitals serving Reading.

You will work in close partnership with partners to develop and implement care plans for frequent hospital attenders.

Where: Reading

Hours: Part Time, 22.5 per week

Salary:£27,861.26 - £32,002.35 (based on full time hours, pro rata for part time) 

Contract Type: Fixed Term unitl 31st March 2026

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

Responsibilities

The Role:

As a valued member of our Reading team you'll be taking on responsibilities including:

  • Work in close partnership with A&E and other departments to develop and implement care plans for frequent hospital attenders.
  • Be that first point of contact for Hospital staff in relation to our service users to ensure seamless delivery of care.

  • Providing specialist assessment of patients attending/admitted to hospital with alcohol related issues using AUDIT screening tools and to provide brief interventions and onward referral for patients.
  • Providing advice, support and training to hospital staff regarding drug and alcohol awareness, screening, assessment processes and referral pathways.
  • Working closely with our community substance misuse teams to ensure assertive and effective follow up of repeat presentations and discharges from the hospital.
  • Keeping GP's informed of patient’s status and discharge plan/referrals within 5 days of patient’s hospital discharge.
  • Developing specific links with identified specialist acute services.
  • Educate and support carers and contribute to the assessment of carers’ needs. 

 

About You:

To thrive in this role, we're looking for someone who has:

  • Excellent understanding of drug and alcohol issues and experience of working within a related field.
  • Training and/or experience in psychosocial intervention
  • Sound working knowledge of safeguarding
  • Sensitivity in working with complex and diverse cultural issues
  • A commitment to multi-agency working
