The Opportunity:

 

We are looking for a Senior Practitioner to join our dynamic team supporting young adults aged 18-25 who are experiencing complex challenges related to substance use. This is a unique opportunity to make a meaningful impact by helping young people access the right support at the right time.

 

You will be based within the Young People’s multi-disciplinary team at ReFresh, working closely with professionals across health, social care, and community services.

 

The role includes outreach work, including home visits and lone working, to ensure accessibility and engagement.

 

Location: ReFresh, Kenworthy House, Hull (with multi-site and home visit responsibilities)

Full-Time Salary Range: £32,002.35 - £34,214.20

Full-Time Hours: 37.5 per week 

Contract Type: Permanent

 

*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. 

 

Responsibilities

Key Responsibilities:

  • Assessment & Engagement:

Conduct comprehensive assessments of service users’ needs, risks, and strengths. Build trusting relationships to support engagement and retention in treatment.

  • Care Planning & Recovery:

Develop and implement personalised recovery plans that are goal-oriented and trauma-informed, with a focus on reducing harm and promoting independence.

  • Operational Leadership:

Act as the operational lead for the 18-25 specialist pathway, ensuring high-quality service delivery and continuous improvement.

  • Supervision & Support:

Provide line management and supervision to 2 x Transitional 18-25 Workers, supporting their development and ensuring effective case management.

  • Multi-Agency Collaboration:

Work in partnership with local services including mental health, housing, education, and criminal justice to provide holistic support.

  • Safeguarding & Risk Management:

Identify and respond to safeguarding concerns, ensuring compliance with local and national safeguarding procedures.

About You:

We’re looking for someone who is:

  • Passionate about supporting young adults and breaking down barriers to engagement.
  • Skilled in trauma informed and person-centred approaches.
  • Experienced in working with complex needs, including substance misuse, mental health and social disadvantage.
  • Confident in leading a small team and managing operational responsibilities.
  • Is passionate about empowering people to make informed choices about their health and well-being.
  • Is confident in developing partnerships to improve service accessibility and engagement.
  • Can provide training and guidance to colleagues, sharing knowledge around harm reduction and interventions.
