Change Grow Live in Walsall are looking for a Senior Practitioner Coordinator to support the development of the Shared care services and improve our relationships with pharmacies.  

 

What are we looking for? The post of Senior Practitioner, you will be the expert in substance misuse providing support to people accessing treatment for opioid dependency. While managing a caseload, the Senior practitioner will play a crucial role in ensuring there is consistency in the delivery of substance misuse treatment across the Opiate team, while at the same time ensuring that our pharmacies and GPs are supported with the people that access our services. The Senior practitioner will be responsible for the delivery of professional training across the Walsall (GPs and Pharmacies) and will deputise in the absence of the team leader.

Where: Wallsall

Hours: Full Time 37.5 hours per week

Salary: £27,861.26 - £32,002.35 Dependent on experience (based on full time hours, pro rata for part time) 

Contract Type: Fixed Term unitl 31st of March 2026 

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role: 

  • Identify, and engage with, people who have substance misuse problems with complex physical and / or mental health needs, including co-morbidity. 
  • Support the development of service-based recovery communities. 
  • Assist and carry out appropriate screening processes including DBST, urine and saliva tests, after completion of appropriate training or demonstration of relevant experience. 
  • Support the Team Leader to carry out case file audits and ensure findings encourage a culture of learning within the team. 
  • Engage in regular consultation with service users to maintain the quality of the service, supporting staff by encouraging service user engagement. 
  • Provide health education especially regarding harm minimisation, blood borne viruses and overdose prevention. 
  • Ensure that all aspects of confidentiality are adhered to. 

 

About you: 

  • Excellent knowledge and understanding of drug and alcohol issues and experience of working within a related field. 
  • Knowledge and experience of multi-agency working and establishing partnerships with other professions to achieve good outcomes for the service user group. 
  • Ability to communicate complex information to individuals and groups in a way that is easily understood. 
  • Written and verbal communication skills and a high degree of personal IT competency. The ability to accurately update and maintain records in a timely fashion and to work to deadlines for the submission of information. 
  • Excellent team working and interpersonal skills, maintaining a highly cooperative approach to supporting colleagues in delivering service objectives.
