Overview

Join Change Grow Live as a Continuity of Care Administrator – and Be Part of Someone’s Journey to Change.

At Change Grow Live, we believe in the potential of people. With the right support, anyone can change direction, grow, and thrive—and we’re here to walk alongside them. If you’re someone who values compassion, collaboration, and creating opportunities for others, we’d love for you to join us.

As a Continuity of Care Administrator, you’ll play a key role in helping people make a smooth and supported transition from custody into the community. Your attention to detail and organisational skills will help ensure no one falls through the cracks, and that individuals continue to receive the care they need to move forward in their recovery.

You’ll be working as part of a skilled and welcoming team who are passionate about making a difference, supporting the national vision set out in the Government’s 10-Year Drug Strategy. Together, we aim to reduce reoffending, improve wellbeing, and create safer, healthier communities.

Based in Ealing, you’ll be part of a vibrant and diverse borough that celebrates its community spirit. The Ealing service is dynamic, inclusive, and rooted in partnership, offering the chance to collaborate with local organisations and make a meaningful impact every day. No two days are the same—and that’s what makes it so fulfilling.
