As a Continuity of Care Administrator, you’ll play a key role in helping people make a smooth and supported transition from custody into the community. Your attention to detail and organisational skills will help ensure no one falls through the cracks, and that individuals continue to receive the care they need to move forward in their recovery.
You’ll be working as part of a skilled and welcoming team who are passionate about making a difference, supporting the national vision set out in the Government’s 10-Year Drug Strategy. Together, we aim to reduce reoffending, improve wellbeing, and create safer, healthier communities.

Full Time Salary Range: £25,077.00 (pro rata for part time hours)
Allowances: £4,133.14 Inner London Weighting (pro rata for part time hours)
Hours: Full-time – 37.5 hours per week
Contract Type: Fixed Term Contract until 31st October 2025, with strong potential for extension based on funding.
*Please note: full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
Acting as the first point of contact for referrals into the service
Facilitating communication between prison, court, police, and community teams
Managing and updating key data and systems
Preparing accurate reports and statistics
Supporting smooth, timely referrals and external transfers
Championing effective, person-centred care coordination

This role requires an Enhanced DBS check and vetting by the Police/HMPPS/National Probation Service. Ongoing clearance will also be required.
