We currently have an exciting opportunity for a Complex Needs Navigator to work in the community as part of a multi-disciplinary teamengaging with a small cohort of service users who are rough sleeping and have complex needs and in close partnership with street-based services including healthcare to assertively engage with people with physical and/or mental health issues and involved in substance use and rough sleeping.

The Navigator will hold a small caseload and provide bespoke support to our Service Users whilst providing a range of flexible and effective interventions to engage with our hard-to-reach Service Users.

Full Time Hours: 37.5 per week

Full Time Salary: £27,449.53 - £31,529.41 pro rata for part time hours (depending on experience) 

Allowances: £4,072.06 Inner London Weighting, Pro Rata for part time hours

Contract Type: Fixed Term until 31st March 2026

Responsibilities

About the role: 

  • Coordinate and lead a solution-based approach, engaging Service Users in appropriate services moving them away from street-based living 
  • Identify and establish contact with rough sleepers and beggars, rapidly assisting Service Users through street outreach shifts at a variety of times and a range of locations, including those unwilling to engage with services 
  • Carry a caseload of complex clients, developing personalised action plans working towards discouragement from rough sleeping 
  • Participate in multi-agency initiatives to reduce the level of street activity 
  • Work closely with the drug and alcohol teams to ensure that substance misusers and problem drinkers can access treatment 
  • Work proactively to ensure that Service Users have access to ongoing healthcare 
  • Participate in the ongoing statistical analysis of rough sleepers and participate in street counts as and when required  

 

About you: 

  • Experience of working in one or more of the following fields: substance misuse, alcohol misuse, mental health 
  • Experience of working with people who have multiple and complex needs 
  • Knowledge and experience of multi-agency working with experience of facilitating multi-agency meetings 
  • Excellent communication, negotiation, and advocacy skills 
  • Be able to build and maintain relationships with a wide range of services 
  • Can work autonomously as well as part of a team 
  • Good levels of IT literacy, including the ability to use IT systems including MS Office applications 
  • Able to critically reflect and evaluate interventions, service delivery and areas for development

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
