Overview

Change Grow Live is a charity dedicated to the belief that we can make a difference to our service users lives, offering support and respect in a safe environment, treating each user as an individual and working with them to find the right support and care options.

 

As the Think Families ‘Family Worker’, you will work as part of a multi-disciplinary team and be based at one of the locality hubs across the County. You will be providing intensive support to families who are currently on Child Protection or Child in Need Plans who have drug and/or alcohol misuse issues which could potentially be putting their children, families or themselves at risk. You will co-ordinate and deliver 1-2-1 interventions, including group based and intensive support packages, enabling families to be able to consider the impacts, identify strengths and re-build their connections to each other in order that vulnerabilities are reduced and that all family members receive the support they need.

  

You will ensure a positive learning environment where integrity and professionalism are paramount, whilst empowering families to ensure all attempts at positive outcomes are achieved for those using the service. You will also develop and maintain excellent working relationships with partners both internally within the multi-agency hub and externally with wider partnerships.

 

 

Where: Norfolk

Hours: Full time, 37.5 per week

Salary: £27,449 - £31,529 dependent on experience (based on full time hours, pro rata for part time) 

Contract: Fixed Term for 5 months

Responsibilities

The Role:
As a valued member of our team in Great Yarmouth, you'll be taking on a central role with responsibilities including:
  • To reduce drug and alcohol related harm to the individual, children, families and wider community
  • To promote healthier life styles and increase service user choice
  • To offer assertive follow up and increased retention
  • To promote and support individuals into both abstinence based and medically assisted forms of recovery
  • To provide effective case management and co- ordination of the individual’s treatment plan
  • To promote carer and service user involvement

 

About You:

To thrive in this role, we're looking for someone who:

  • Demonstrates a solid understanding of substance misuse issues and has experience in this or a related field
  • Possesses excellent communication skills, both verbal and written, coupled with proficient IT skills
  • Takes a proactive approach, working well independently with strong time management and ability to prioritise work
  • Holds a good working knowledge of mental health interventions, services, and best practices
  • Is a supportive team player with strong interpersonal skills, capable of working collaboratively with a diverse range of professionals, agencies, and internal/external stakeholders
  • Can successfully manage change while prioritising the needs of those who use our service
  • Actively seeks learning opportunities to enhance professional knowledge and skills, contributing to the learning and development of others.
  • To undertake assessments which meet specified standards and timescales
  • Organising and managing packages of support to enable service users to lead the fullest lives possible
  • To provide recovery and family coaching using a broad range of psychosocial interventions
  • To work in partnership with community pharmacists ensuring clear communication pathways in relation to medication and supervised consumption.
