We have an exciting opportunity for a Primary Care Recovery Coordinator who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.  

 

 

Where: Southampton

Full Time Hours: 37.5 per week 

Full Time Salary: £27,449.53 - £31,529.41 (pro rata for part time hours)

Contract: Fixed Term until 31st March 2025

Responsibilities

About the role: 

 

  • Accept referrals, assess need and make appropriate recommendations for care.
  • Consult with Primary Care (GP’s), community mental health services, mental health hospitals, IAPT services and other providers of mental health and substance use support to give services users a robust package of care to support their recovery.
  • Be responsible for maintaining a full case load of service users with complex needs.
  • Conduct comprehensive assessments, construct care packages with multi-disciplinary partners where required, review care plans in a structured format and manage onward referrals.
  • Conduct home visits to service users to support those with disabilities and to those with family responsibilities.
  • Develop and sustain links with multi-disciplinary including statutory and non-statutory providers, Criminal Justice and wrap around services.

 

About you: 

 

  • Excellent knowledge and understanding of drug and alcohol issues and experience of working within a related field.
  • Excellent knowledge and understanding of prescription drug dependence, related health and co-morbidities, or ability and willingness to learn.
  • Experience of working with clients with complex physical and/ or mental health needs including co-morbidity.
  • Experience of working in a highly pressurised environment and the ability to use strategies to prioritise and manage work-load.
  • Excellent team working and interpersonal skills, maintaining a highly cooperative approach to supporting colleagues in delivering service objectives.
