Responsibilities

About the role:

In this role, you will have the opportunity to support individuals with co-existing mental health and substance use challenges, who may also experience additional complexities such as neurodevelopmental conditions, learning disabilities, physical health issues, lack of community or family support, or unsuitable living conditions. You will work to overcome barriers to engagement, helping individuals connect with meaningful support, and addressing safeguarding and exploitation concerns.

Working as an integral part of a compassionate, multi-disciplinary team that includes Recovery Practitioners, Doctors, Nurses, Psychiatrists, and Psychologists, you will support each individual’s journey to recovery from their initial contact with RISE through to ongoing treatment.

Your role will include:

  • Conducting comprehensive assessments and screenings
  • Offering tailored pharmacological and psychosocial interventions, including cognitive-behavioural approaches, motivational interviewing, and recovery planning
  • Providing support that reduces drug and alcohol-related harm, and fosters greater involvement from family, carers, and the wider community
  • Making onward referrals that connect individuals with the right services for their continued recovery

You will work flexibly across both community and inpatient settings, using an assertive outreach approach to meet people where they are. This includes visiting individuals in locations such as Acute Psychiatric Wards, Substance Misuse Hubs, Community Mental Health Services, GP Surgeries, and Supported Accommodation.

The post holder will need to work flexibly across the local community and inpatient settinings with hours to suit service need.
The team is peripatetic rather than hub based with the post holder expected to conduct assessments & reviews using assertive in-reach in a variety of venues – including Acute Psychiatric Wards, Substance Misuse Hubs, Community Mental Health Services, GP Surgeries, and Supported Accommodation.

About You:

  • You will bring empathy, patience, and a strong commitment to person-centred support and treatment.
  • You are flexible, with the ability to work across different community settings, and adapt your approach to meet the unique needs of each individual.
  • You have experience in supporting people with complex mental health and substance use challenges and are skilled at building trust and encouraging positive change.
  • Your approach is inclusive and trauma-informed, ensuring that the people you work with feel heard, valued, and empowered.
