An integrated pathway from prison to community is vital in reducing reoffending and supporting recovery from drug and alcohol dependency for those leaving custody. The continuity of care between prisons and community support services is a key focus in the government’s 10-Year Drug Strategy, and this role is integral to ensuring that seamless transition. To help us achieve this in Ealing we are looking for a dedicated Continuity of Care Administrator to join our vibrant team.

 

Hours: Full Time - 37.5hrs per week

Full Time Salary Range£23,400.00 - £23,685.20 (pro rata for part time hours)

Allowances: Inner London Weighting £4,072.06

 

Responsibilities

About the Role:

In this role, you will be the key contact for referrals, coordinating with stakeholders, updating local data systems, and compiling and submitting statistical information. You will:

  • Lead daily communications and information sharing between prison and community teams.
  • Monitor and manage incoming referrals and assist with external transfers.
  • Collaborate with both prison, courts, police and community teams to ensure that administration and systems run smoothly and efficiently.
  • Maintain accurate and up-to-date data, performing regular checks for errors and correcting them as needed.
  • Provide clear and concise advice, information, and reports through the most appropriate channels.
  • Input, collate, and extract data from Change Grow Live’s internal database, working with multiple case management systems.

  • Has experience working in an office environment with responsibilities for administration.
  • Is experienced in managing data systems.
  • Demonstrates a positive, solution-oriented, and flexible approach to their work.
  • Maintains an empathetic and non-judgmental attitude towards others.
