We have an exciting opportunity for a Young Persons Transitional Worker who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.  

 

The post holder will engage young people in a range of interventions, utilizing strengths based and young person-centered approaches, empowering them to reduce risk, increase resilience and lead a safe, happy, and healthy life.

 

No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key. 

 

Where: Croydon

Full Time Hours: 37.5hrs per week

Full Time Salary: £27,449 - £31,529 dependent on experience 

Allowance: Outer London Weighting of £2163.27 pro rata 

Responsibilities

About the role:

 

  • Engage young people who have substance use issues and provide them with advice, support, and structured interventions to support their health and wellbeing needs
  • Working with clients between the age of 18 and 25 supporting the transition between young people’s services and adult services.
  • Work and liaise with specific young adults agencies involved with the young adult’s broader care plan.
  • Complete comprehensive screening and assessments and develop individual care plans that incorporate integrated care and transition support
  • Promoting young adults and young people’s participation and co-production opportunities to support identification of potential gaps in provision and unmet needs
  • Work closely within the 16+ supported accommodation Multi-Disciplinary Team model to provide a co-ordinated, flexible and responsive approach to support

 

About you:

  • Knowledge and experience of providing interventions for service users affected by Substance use.
  • Be an excellent communicator, both verbal and written with good IT skills.
  • Proactive and able to work on own initiative with excellent time management and prioritising skills.
  • Have a good working knowledge of mental health interventions, services, and good practice.
  • Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders.
  • Ability to manage change successfully in a way that prioritises the needs of service users.
  • Understand the importance of information governance processes and commit to follow and apply all necessary safeguards.
  • Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others.
