We have an exciting opportunity for a  Shared Care Recovery Coordinator to work in local GP surgeries as part of our busy Tower Hamlets service, delivering substance misuse interventions in community settings with a focus on Alcohol interventions. 

 

You’ll have to record information about what you do, so enjoying a bit of paperwork and attention to detail is important. But most of the time you’ll be enjoying working with people from all backgrounds, making a difference to their lives. No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key.

 

Where: Tower Hamlets, London

Full Time Hours: 37.5hrs per week

Full Time Salary: £27,449.53 - £31,529.41 dependent on experience, Pro rata

Allowance: £4,072 Inner London Weighting pro rata

Contract Type: Fixed Term until March 2025

 

 

Responsibilities

About the role:

 

  • Supporting service users from point of entry into the service and through their treatment/recovery journey
  • Work with the Shared Care GP to support service users to achieve sustainable recovery
  • Communicate with local GPSI's where applicable in relation to substance misuse treatment in Primary Care
  • Work in partnership with community Pharmacists, ensuring clear communication pathways in relation to supervised consumption arrangements, engagement/presentation and medication collection
  • Promote sexual health awareness, provide advice and information on protective factors and reducing risks and offer screening in association with the Practice Nurse
  • Provide screening, assessment, and recovery planning and onward referral
  • Reduce drug and alcohol related harm to service users and the wider community
  • Promote carer, service user and community involvement
  • Provide advocacy for access to partnership services
  • Work with service users to support social (re)integration, enabling them to lead meaningful and purposeful lives: promoting recovery, resilience, peer support and self-determination

 

About you:

 

  • Have a strong understanding of substance misuse issues and experience of working within a similar field
  • Sound and demonstrable knowledge of clinical prescribing issues
  • Experience of community based support practices
  • Be an excellent communicator, both verbal and written with good IT skills
  • Proactive and able to work on own initiative with excellent time management and prioritising skills
  • Have a good working knowledge of mental health interventions, services, and good practice
  • Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
  • Ability to manage change successfully in a way that prioritises the needs of service users
  • Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
  • Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
