As a Recovery Coordinator with Change Grow Live you’ll work with people who use our services, helping them get to where they want to be. This role is part of the new Dependence Forming Medications (DFM) Team.

The DFM team support people who have a dependency to Opiates/Opioids, Benzodiazepines, GabaMedicines, or Z-drugs. Working closely with individuals and liaising with health services and prescriber's you will have a variety of bases within GP surgeries and our main hub.

An ability to engage well with people despite a range of barriers is vital, helping service users/patients overcome fear and ambivalence, and building trusting professional relations.

Hours: 37.5 per week 

Salary: £27,449.53 - £31,529.41 

Responsibilities

About the role: 

 

  • As part of a MDT, identify, and engage with, people who have prescription medication dependence problems with complex physical and/ or mental health needs, including co-morbidity.
  • Build therapeutic relationships and deliver a tailor-made package of care including Recognised psycho-social interventions, 1:1 key work, POD and group work.
  • Carry out comprehensive assessments and develop recovery plans that clearly identify how they will enable progression in addressing substance/medication use and re/integrating into the community where applicable. 
  • Support the GPs and the MDT in implementing strategies for managing challenging, aggressive, or abusive patients. 
  • Refer patients who require support for complex poly-substance use into West Sussex DAWN.

 

 

About you: 

 

  • Excellent knowledge and understanding of drug and alcohol issues and experience of working within a related field.
  • Excellent knowledge and understanding of prescription drug dependence, related health and co-morbidities, or ability and willingness to learn.
  • Experience of working in a highly pressurised environment and the ability to use strategies to prioritise and manage work-load.
  • Experience of working with clients with complex physical and/ or mental health needs including co-morbidity.
