We have an exciting opportunity for a Aftercare Practitioner who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs. As an Aftercare Practitioner with Change Grow Live you’ll work with people who use our services and help support them into transitioning back into local communities and developing their social and local resources. This will support long term recovery.

 

With this role you’ll be working as part of Recovery team, based in Barnet. The team supports people with re-intergration back into local communities after treatment, and focuses on supporting people to build social and local resources, that builds a foundation of recovery capital.

 

 

No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key.

 

You’ll have to record information about what you do, so enjoying a bit of paperwork and attention to detail is important. But most of the time you’ll be enjoying working with people from all backgrounds, making a difference to their lives.

 

Where: Edgware

Hours: 37.5 per week

Salary: £26,393.77 - £30,316.74 dependent on experience

Allowances: £2,163.27 Outer London Weighting

Responsibilities

About the role: 

 

  • Supporting service users in Aftercare/Recovery Support through their remaining time in treatment with the service.
  • Providing 1-1 sessions and structured group work programme and recovery planning and onward referrals to specialist services.
  • Reducing drug and alcohol related harm to service users and the wider community
  • Work with prescribing clinicians to support substitute prescribing regimes to improve social functioning.
  •  Aftercare care coordinator overseeing assigned groups of service users, providing: 1-1s; structured group work; asset mapping; recovery planning (using motivational interviewing techniques) and reviews of care.

 

About you: 

 

  • Excellent understanding of drug and alcohol issues and experience of working within a related field.
  • Accomplished written and verbal communication skills and a high degree of personal IT competency.
  • An understanding of and commitment to safeguarding best practice.
  • Effective interpersonal skills with the ability to engage successfully with internal and external stakeholders at all levels.
  • Excellent time management skills, and an ability to work on own initiative, prioritisingaccordingly.
